NOTICE TO PATIENTS
PLEASE CAREFULLY READ BEFORE ENTERING THE CLINIC
IN ORDER TO PROTECT OUR PATIENTS AND STAFF, AND TO PREVENT THE FURTHER SPREAD OF THE CORONAVIRUS (COVID-19), WE RESPECTFULLY REQUEST THAT YOU FOLLOW THE FOLLOWING PRECAUTIONS AND GUIDELINES:

If you have a fever or feel feverish AND have a dry cough, PLEASE go back to your car and make an appointment with your primary care provider or an Urgent Care facility so you can be evaluated, treated and/or tested for the Coronavirus (COVID-19).

If any of the following apply to you, PLEASE do not keep your appointment today, PLEASE do not enter the office and PLEASE reschedule your appointment at least 14 days after your symptoms have resolved or 4 weeks after exposure or an overseas trip:

  • Cough, fever, chills, runny nose, stuffy nose, sore throat;
  • You have traveled overseas in the past 30 days;
  • You have been exposed to anyone who has traveled overseas in the past 30 days; or
  • You have been exposed to anyone diagnosed with the Coronavirus (COVID-19).

If you have any concerns about your symptoms or possible exposure to the Coronavirus (COVID-19), please immediately contact your primary care provider or seek treatment from an Urgent Care facility.

Please note that ONLY THE PATIENT will be allowed into the office, unless a parent accompanying a child or there are special circumstances. All other family/friends/co-workers should remain outside or in the car.

We reserve the right to take your temperature upon entering the office. If you have a fever you will be asked to reschedule your appointment.

If we have a high suspicion for COVID-19, our provider may recommend that you leave the office and go to the ER or another facility for further testing and/or treatment.

Please note that we do not have rapid Coronavirus testing at our facility.

Please thoroughly wash your hands for at least 20 seconds with soap and water and/or use hand sanitizer when entering the office. Avoid touching your eyes, nose and mouth with unwashed hands.

Please cough or sneeze into your elbow and immediately re-wash your hands after coughing or sneezing.

Please maintain social distancing (at least 6 feet distance) from other patients and staff at all times while in the office.

IF YOU HAVE FEVER AND A COUGH, AND ARE EXPERIENCING SHORTNESS OF BREATH OR CHEST PAIN, PLEASE GO DIRECTLY TO THE CLOSEST ER OR DIAL 911.
